This evidence-based health assessment was developed for Australians and assesses your risk of developing chronic conditions such as diabetes and heart disease. The results will assist our Health Care Services team and your doctor to recommend a range of health and wellbeing resources and programs for you.

This questionnaire is hosted and published by Queensland Teachers’ Union Health Fund Limited (TUH / Union Health) (ACN 085 150 376) of 438 St Pauls Terrace, Fortitude Valley, QLD, 4006. TUH Health Care Services Pty Ltd (HCS), a wholly-owned subsidiary, assists in data processing.
All adults on any cover may complete the Health-e-Profile. We pay an Active Health Bonus to members on any combined hospital and extras products except those with Basic Extras (combined with any hospital product) if they complete the Health-e-Profile every 12 months. To claim the Active Health Bonus (on eligible covers), the Health-e-Profile must be completed by any adult on the cover, other than an adult dependant. You can use the Active Health Bonus to pay out-of-pocket costs on your extras services.
