Health support programs
Health-e-Profile
This online assessment helps you spot risk factors that may lead to chronic health conditions. If risk factors are found, you may receive health information to assist you in reducing your risk or if you receive a moderate to high risk score one of our Health Care Services team may contact you to discuss specific health resources and programs to help you stay healthy and reduce your risk of developing these conditions.
All adults on any cover may complete the Health-e-Profile.
We pay an Active Health Bonus to members on any combined hospital and extras products except those with Basic Extras (comined with any hospital product. if they complete the Health-e-Profile every 12 months. To claim the Active Health Bonus, the Health-e-Profile must be completed by any adult on the cover, other than an adult dependant. You can use the Active Health Bonus to pay out-of-pocket costs on your extras services.

Hospital substitute programs
We have contracts with a number of providers of programs that provide tailored treatment options to keep you out of hospital or help you make a speedy recovery at home after illness or an operation.
How does it work?
Before or during your hospital stay tell your doctor and nursing staff that you can access our programs. They’ll assess your situation and advise if the program is medically appropriate for you. With your consent, your doctor or nursing staff will contact a program coordinator (registered nurse), who works closely with you and the hospital to organise the appropriate services.

Palliative care
Negative pressure therapy
Orthopaedic rehabilitation program
Respiratory rehabilitation program
Cardiac rehabilitation program
Reconditioning rehabilitation program
If you have hospital cover you can participate in a program so long as your cover includes benefits for that treatment or procedure. A waiting period of two months applies.
The programs are available throughout our participating hospital network. Union Health is not involved in any clinical assessment.

Disease management programs
The disease management programs can help you manage chronic health conditions with expert telephone support tailored to your health needs.
Remedy Health Coaching offers one-on-one coaching sessions over the phone, designed to suit individual preferences and health goals. The dedicated coaches include dieticians, nurses, diabetes educators, exercise physiologists and occupational therapists.
The program is designed to complement services provided by your existing health providers. A health coach will call you, discuss your needs and recommend a suitable program. They will keep in contact with your treating doctor to monitor your progress.
Disease management programs are available to all Union Health members with hospital cover. A six-month waiting period may apply.

Healthy weight for life
We offer Healthy Weight for Life for eligible members. The program is designed to reduce risk factors related to diagnosed osteoarthritis, diabetes and heart health as well as prevent or delay complications occurring from these diseases. The program is centred on the clinically proven intensive Kick Start™ Very Low Calorie Diet provided along with dedicated health coaching and online support.

Cancer Support Complete Program
We’ve partnered with Valion Health to fund a Cancer Support Complete Program for members who have had a cancer diagnosis.
The program supports the physical and mental health of members who are undergoing, or have recently completed, cancer treatment and those living with advanced cancer. Individual care plans may include support from a cancer nurse, dietitian, exercise physiologist, psychologist and yoga therapist over 12-weeks.
Mental health
For telephone and online mental health support we recommend contacting beyondblue on 1300 22 4636. beyondblue provides a free 24 hours/7 days a week phone service where trained mental health professionals are available for confidential support. Beyondblue also provides an online chat service between 3pm and 12am 7 days a week.
For suicide and crisis support, we recommend contacting Lifeline on 13 11 14.
